Custom print your company name and address (up to five lines) on these 8 1/2" tall x 11" wide, 4-part carbon forms. These forms provide plenty of space for listing repair instructions, parts used and labor time. Part 3 includes the word 'Estimate" printed in red across the instruction area. Additional materials and outside work changes can be listed on the backs of part 1, 2 and 3. the back of part 4 has areas for additional materials and labor, outside parts and a labor record (time card) printed on manila or buff board. These forms are consecutively numbered (you choose the starting number). Two versions available: California (meets the legal requirement for the state) and Non-California. Minimum order quantity is 250. Quantity Discount Prices available on quantities of 500 and 1,000.
